Travel Nurse Contract Sample

Parties:

Travel Nurse: Emily Johnson

Healthcare Facility: Sunlight Medical Center

Assignment Details:

The traveler agrees to provide nursing services at the Facility from __________________ to __________________. The position involves providing patient care in accordance with the facility’s policies and applicable regulations.

Compensation and Benefits:

The traveler will receive an hourly rate of $50, payable bi-weekly via direct deposit. Additional benefits include housing allowance, travel reimbursement, and malpractice insurance, as agreed.

Responsibilities:

Termination:
